Legal
Terms of Service
These terms explain the rules for using FormLevel, who owns what, and the limits of fitness and AI-generated guidance.
Effective and last updated July 19, 2026
1. Agreement and eligibility
These Terms of Service form an agreement between you and the person or entity operating FormLevel (the "FormLevel operator"). By accessing the service, entering a beta PIN, uploading a video, or generating a plan, you agree to these terms and acknowledge the Privacy Policy and Fitness Disclaimer.
You must be at least 13 years old. If you are under the age of legal majority where you live, a parent or legal guardian must review and agree to these terms for you. Do not use FormLevel if you do not agree.
2. The beta service
FormLevel is an early-stage fitness tool for exercise form analysis, workout planning, and general meal planning. Accounts, guided-workout progress, estimated starting weights, check-ins, and rewards are beta features. Features may be incomplete, unavailable, inaccurate, changed, reset, or removed without notice. A shared beta PIN grants temporary access; it is not a personal account and should not be posted publicly. A separate verified Supabase account can save your profile, workout history, progress, XP, and support requests. Guest use does not create cloud history.
The current detailed form-analysis workflow is strongest for squats. Recognition or demonstration of another exercise does not mean detailed scoring is available for that movement.
3. Fitness and AI guidance
FormLevel provides general educational information, not medical care, diagnosis, treatment, physical therapy, or individualized advice from a qualified coach, dietitian, or clinician. Computer vision and AI can produce incomplete or incorrect results.
You remain responsible for deciding whether an exercise, workout, or food suggestion is appropriate for you. A suggested dumbbell range is a conservative estimate based on limited multiple-choice answers and exercise metadata. It is not a measured strength result, a one-repetition maximum, or a promise that a load is safe for you. Use a lighter weight whenever you are unsure or cannot maintain control.
Per-exercise "too heavy" and "too light" responses may influence later workout days, but FormLevel does not monitor you in real time or confirm that your selection is accurate. Review the fullFitness Disclaimerbefore relying on any output.
4. Your videos and information
You keep any ownership rights you have in videos, support messages, screenshots, and other information you submit. You give the FormLevel operator a limited, non-exclusive license to host, copy, process, transform, and display that material only as needed to operate, secure, troubleshoot, and provide the requested service.
You confirm that you have permission to upload the recording and that every identifiable person in it has consented. Do not upload confidential material, recordings of another person without permission, or content that violates privacy, intellectual-property, or other rights.
5. Acceptable use
You may not:
- use FormLevel for unlawful, harmful, fraudulent, or abusive activity;
- upload malware, corrupted media intended to attack the service, or content you do not have a right to use;
- bypass access controls, rate limits, usage limits, or security protections;
- probe, disrupt, overload, scrape, or reverse engineer the service except where applicable law expressly permits it;
- resell beta access, generated plans, or licensed demonstration media; or
- represent FormLevel output as a medical diagnosis or guaranteed professional assessment.
6. FormLevel rights and trademarks
The service's software, interface, original text, graphics, analysis presentation, and branding are owned by the FormLevel operator or used under license. Subject to these terms, you receive only a limited, revocable right to use the service for personal evaluation during the beta.
Copyright in FormLevel's original site content and software is claimed by the FormLevel operator. © 2026 FormLevel. All rights reserved, except for rights expressly granted in these terms.
FormLevel™ and related branding are claimed as trademarks of the FormLevel operator. The ™ symbol states a trademark claim; it does not represent federal registration. Do not use confusingly similar branding or imply sponsorship or endorsement without written permission.
Exercise demonstration videos, posters, names, and other third-party materials remain the property of their respective owners or licensors. Access through FormLevel does not grant you permission to download, redistribute, sell, or reuse those assets outside the service.
7. Privacy and service providers
The Privacy Policyexplains what FormLevel processes, why it is used, how long temporary media is kept, and which service providers support the product. By using FormLevel, you acknowledge those practices.
8. Availability, suspension, and changes
FormLevel may limit, suspend, or end access to protect users, control cost, investigate misuse, maintain the service, or discontinue the beta. The operator may update these terms by publishing a revised date. Guest plans and temporary workout state may be lost if site data is cleared, a browser blocks storage, or the feature changes. Signed-in history and progress use cloud storage but may still be affected by outages, account deletion, beta resets, or service changes. If a material change is made, reasonable notice will be provided in the service when practical.
9. Disclaimers and liability limits
To the fullest extent permitted by law, FormLevel is provided "as is" and "as available," without warranties of accuracy, availability, fitness for a particular purpose, non-infringement, or results. Nothing in these terms excludes a warranty or consumer right that applicable law does not allow to be excluded.
To the fullest extent permitted by law, the FormLevel operator and contributors are not liable for indirect, incidental, special, consequential, or punitive damages, lost data, lost profits, personal training decisions, equipment choices, load selections, or harm caused by relying on an output. These limits do not apply where prohibited by law or to liability that cannot legally be limited.
10. Applicable law
Applicable local, state, federal, and international laws continue to apply. These terms do not waive any non-waivable rights you have under the law where you live. A governing-law or dispute-resolution clause may be added before a wider commercial launch once the operating legal entity and jurisdiction are set.
